“Puerto Rico Medicaid Incentive Program Registration & Attestation website is now open
The PSM-HITREC informs our provider’s and hospital in Puerto Rico that the Puerto Rico State Level Registry (PR-SLR) is open to apply for the American Recovery and Reinvestment Act (ARRA) Medicaid E.H.R. Incentives.
The PR-SLR can be accessed at http://pr.arraincentive.com/
The Providers and Eligible Hospitals' Checklist to Getting Started:
[ ] Computer
[ ] Access to the Internet
[ ] Complete the CMS EHR Incentive Program Registration
[ ] Contact name, phone and email address
[ ] Pen and pad to write down created username and password
[ ] Printer and Scanner or Right Fax
[ ] Optimal Web browser to use is Internet Explorer 8.0 and above
There are some REQUIRED DOCUMENTS AND KEY INFORMATION for the Providers and Eligible Hospitals:
[ ] EPs and EHs Step 1 - Electronic copy of License, Good Faith Certification or Certification ID
[ ] EPs and EHs Step 2 - Know the 90 Day Representative Period in the previous calendar year you would like to use
[ ] EPs Step 2 - Know the unique Total and Medicaid Patient Encounter for the 90 day period you selected; 1 encounter is a unique patient visit per day
[ ] EPs Step 2 - Electronic copy of practice management report for the 90 day period identified
[ ] EHs Step 2 - Know the total discharges for the full hospital fiscal year and total patient bed days for a full hospital fiscal year to calculate average length of stay
[ ] EHs Step 2 - Know the Total and Medicaid Discharge data for the 90 day period identified
[ ] EHs only - Electronic copy of the 4 years of cost report data and Hospital Calculation worksheet
[ ] Contract for E.H.R. Incentive software signed by the provider and the E.H.R. vendor and dated. (It is required to attach this document to Step 3a)
